The starting point

The plant makes fresh, water-packed tofu — soak, grind, cook, coagulate, press, cut, pack, chill — on three lines, shipping to grocery and food-service across the West Coast. Fresh tofu is the hardest kind to make safely: high-moisture, low-acid, protein-rich, ready-to-eat, with a short cold shelf life. It is, in other words, an ideal home for Listeria monocytogenes. Sanitation was the whole game, and it ran on paper: pre-op checklists on clipboards, swab results in a binder, coagulation and fill checks in a logbook nobody read twice.

Then the environmental-monitoring program started catching it: an intermittent Listeria-genus positive on a floor drain near the packing room. Never on product, always on the same zone, never explained. Each hit triggered the same scramble — intensified cleaning, re-swab, a corrective-action form filed and forgotten — and a month or two later it came back. Everyone knew that a single product-contact positive, or one FDA swab on a bad day, could mean a recall that ends the business.

What changed, step by step

Step 1 · Audit · 5S

Pre-op sanitation, on a phone instead of a clipboard

The team mapped the plant into 18 hygiene zones — soak tanks, grinders, cook kettles, the coagulation deck, presses and cloths, the cutting line, the ready-to-eat packing room, cold storage, drains and washdown perimeters. Pre-op and GMP rounds now run as phone walks with photo evidence: standing water, a frayed press cloth, a hose lying on the floor of an RTE zone, condensate over an open tank — each is a timestamped picture, not a checkmark. Out-of-tolerance readings on a checklist auto-create a defect, so a failed pre-op can't be signed off and buried until the next audit.

Step 2 · Defects

Hygiene, maintenance, and food-safety findings on one board

Everything lands on the same board — a cracked floor tile channeling washdown water, biofilm on a drain grate, a worn door seal on cold storage, a coagulation tank running out of temperature band. The AI tags the likely category from the photo and clusters repeats. For the first time it put a number on what QA suspected: the packing-room drain zone generated more sanitation findings than any other area in the plant, month after month.

Step 3 · Fix-Plan

Closures with photo proof, before the swab comes back

Every finding carries an owner, a due date, and an after-photo requirement. The AI drafts fix-plans from what closed similar findings before — for the door seal, the gasket spec and lead time from the last occurrence. Median time from finding to verified closure fell from 16 days to 4, and overdue food-safety actions — the first thing any auditor counts — dropped to zero.

Step 4 · Root cause · A3

The recurring Listeria positive finally got a real A3

The drain-zone positive became an A3, with the lab's environmental-monitoring reports and the trend chart attached as case context. The fishbone crossed Machine, Method and Environment; the 5-Why thread walked from 'positive on the drain' down to a hairline-cracked drain body under the floor that was harboring a biofilm no surface cleaning could reach — kept alive by a washdown pattern that ran water from the packing zone back toward it. Countermeasures: replace the drain with a hygienic-design unit, re-sequence washdown so RTE zones drain away from product, add a zoned footwear-and-hose rule, and put a targeted swab on the repaired area. Verified over three months of monitoring — the plant has not had a Listeria positive in that zone, or anywhere in the RTE area, in the twelve months since.

Step 5 · Kaizen · savings

Soybean yield and utilities became the savings line

With coagulation and fill now measured instead of eyeballed, the kaizen wave had a baseline. Standardizing coagulant dosing and cook temperature cut firmness rejects and the block breakage that fed rework; an okara-and-whey recovery project clawed back protein that had been going down the drain; water and steam reclamation on the cook line trimmed two of the plant's biggest utility bills. The verified ledger — expected at approval, actuals signed off by finance — reached $260,000 a year, with soybean yield the single biggest line in a business where the bean is the cost.

Step 6 · OPL → Skill matrix

Sanitation standards that train the next hire

The drain A3 and the coagulation standard became one-point lessons — photo-first, in English and Spanish for a bilingual floor. Acknowledgement updates the skill matrix, so QA can show an auditor exactly which of the 140 employees is current on RTE-zone sanitation, allergen changeover, and Listeria control. New line and sanitation hires now train against the same OPLs in their first week, instead of learning the drain the hard way.

Step 7 · Leaderboard

The sanitation shift stopped being invisible

Zone hygiene scores, closed findings and clean swab streaks all earn points, and the third shift — the overnight sanitation crew that the day never sees — now tops the board most weeks. The plant manager opens the daily stand-up with the scoreboard and the clean-swab streak instead of the complaint list. Near-miss and hygiene-issue reporting tripled once logging one took thirty seconds, which QA counts as the real culture change: problems surface while they're still cheap.
